SUBMIT APPLICATION
Phase 1: Complete the Online Application
In order to attend QUEST all students entering Grades 5 and 6 in September must complete the Application Form. The open enrollment application for the 2021-2022 school year must be submitted between January and April 2022.
Applications will not be accepted after the deadline unless the family is new to Bermuda or a family student who has a change of residence and therefore needs to complete another application form. Proof or change of residence is required.
QUEST APPLICATION PACKAGE
The following items are required to be submitted on behalf of a student interested in attending QUEST:

Application/Nomination Form - in writing

Standardized Assessment (Ability or Achievement Test) Report or Audition

Behaviour Checklist(s) - completed by a qualified teacher/mentor/coach only (see below to print for completion)

Student’s recent Report Card (grades 90% or above in nominated discipline)

Portfolio - body of work to show evidence of student’s performance (i.e., DVD compilation, Art work, Programs of Events participated in, Recital videos, Mathematics, Science, Language Arts Test)

Copy of the Child's Birth Certificate

If applicable, the Parent's and/or Legal Guardian(s) Work Permit, or Proof of Permanent Residency
